In Medicine, the Morally Unthinkable Too Easily Comes to Seem Normal

Here is the way I remember it: The year is 1985, and a few medical students are gathered around an operating table where an anesthetized woman has been prepared for surgery. The attending physician, a gynecologist, asks the group: “Has everyone felt a cervix? Here’s your chance.” One after another, we take turns inserting two gloved fingers into the unconscious woman’s vagina.

Had the woman consented to a pelvic exam? Did she understand that when the lights went dim she would be treated like a clinical practice dummy, her genitalia palpated by a succession of untrained hands? I don’t know. Like most medical students, I just did as I was told.

Last month the Department of Health and Human Services issued new guidance requiring written informed consent for pelvic exams and other intimate procedures performed under anesthesia. Much of the force behind the new requirement came from distressed medical students who saw these pelvic exams as wrong and summoned the courage to speak out.

Whether the guidance will actually change clinical practice I don’t know. Medical traditions are notoriously difficult to uproot, and academic medicine does not easily tolerate ethical dissent. I doubt the medical profession can be trusted to reform itself.

What is it that leads a rare individual to say no to practices that are deceptive, exploitative or harmful when everyone else thinks they are fine? For a long time I assumed that saying no was mainly an issue of moral courage. The relevant question was: If you are a witness to wrongdoing, will you be brave enough to speak out?

But then I started talking to insiders who had blown the whistle on abusive medical research. Soon I realized that I had overlooked the importance of moral perception. Before you decide to speak out about wrongdoing, you have to recognize it for what it is.

This is not as simple as it seems. Part of what makes medical training so unsettling is how often you are thrust into situations in which you don’t really know how to behave. Nothing in your life up to that point has prepared you to dissect a cadaver, perform a rectal exam or deliver a baby. Never before have you seen a psychotic patient involuntarily sedated and strapped to a bed or a brain-dead body wheeled out of a hospital room to have its organs harvested for transplantation. Your initial reaction is often a combination of revulsion, anxiety and self-consciousness.

To embark on a career in medicine is like moving to a foreign country where you do not understand the customs, rituals, manners or language. Your main concern on arrival is how to fit in and avoid causing offense. This is true even if the local customs seem backward or cruel. What’s more, this particular country has an authoritarian government and a rigid status hierarchy where dissent is not just discouraged but also punished. Living happily in this country requires convincing yourself that whatever discomfort you feel comes from your own ignorance and lack of experience. Over time, you learn how to assimilate. You may even come to laugh at how naïve you were when you first arrived.

A rare few people hang onto that discomfort and learn from it. When Michael Wilkins and William Bronston started working at the Willowbrook State School in Staten Island as young doctors in the early 1970s, they found thousands of mentally disabled children condemned to the most horrific conditions imaginable: naked children rocking and moaning on concrete floors in puddles of their own urine; an overpowering stench of illness and filth; a research unit where children were deliberately infected with hepatitis A and B.

“It was truly an American concentration camp,” Dr. Bronston told me. Yet when he and Dr. Wilkins tried to enlist Willowbrook doctors and nurses to reform the institution, they were met with indifference or hostility. It seemed as if no one else on the medical staff could see what they saw. It was only when Dr. Wilkins went to a reporter and showed the world what was happening behind the Willowbrook walls that anything began to change.

When I asked Dr. Bronston how it was possible for doctors and nurses to work at Willowbrook without seeing it as a crime scene, he told me it began with the way the institution was structured and organized. “Medically secured, medically managed, doctor-validated,” he said. Medical professionals just accommodated themselves to the status quo. “You get with the program because that’s what you’re being hired to do,” he said.

One of the great mysteries of human behavior is how institutions create social worlds where unthinkable practices come to seem normal. This is as true of academic medical centers as it is of prisons and military units. When we are told about a horrific medical research scandal, we assume that we would see it just as the whistle-blower Peter Buxtun saw the Tuskegee syphilis study : an abuse so shocking that only a sociopath could fail to perceive it.

Yet it rarely happens this way. It took Mr. Buxtun seven years to convince others to see the abuses for what they were. It has taken other whistle-blowers even longer. Even when the outside world condemns a practice, medical institutions typically insist that the outsiders don’t really understand.

According to Irving Janis, a Yale psychologist who popularized the notion of groupthink, the forces of social conformity are especially powerful in organizations that are driven by a deep sense of moral purpose. If the aims of the organization are righteous, its members feel, it is wrong to put barriers in the way.

This observation helps explain why academic medicine not only defends researchers accused of wrongdoing but also sometimes rewards them. Many of the researchers responsible for the most notorious abuses in recent medical history — the Tuskegee syphilis study, the Willowbrook hepatitis studies, the Cincinnati radiation studies , the Holmesburg prison studies — were celebrated with professional accolades even after the abuses were first called out.

The culture of medicine is notoriously resistant to change. During the 1970s, it was thought that the solution to medical misconduct was formal education in ethics. Major academic medical centers began establishing bioethics centers and programs throughout the 1980s and ’90s, and today virtually every medical school in the country requires ethics training.

Yet it is debatable whether that training has had any effect. Many of the most egregious ethical abuses in recent decades have taken place in medical centers with prominent bioethics programs, such as the University of Pennsylvania , Duke University , Columbia University and Johns Hopkins University , as well as my own institution, the University of Minnesota .

One could be forgiven for concluding that the only way the culture of medicine will change is if changes are forced on it from the outside — by oversight bodies, legislators or litigators. For example, many states have responded to the controversy over pelvic exams by passing laws banning the practice unless the patient has explicitly given consent.

You may find it hard to understand how pelvic exams on unconscious women without their consent could seem like anything but a terrible invasion. Yet a central aim of medical training is to transform your sensibility. You are taught to steel yourself against your natural emotional reactions to death and disfigurement; to set aside your customary views about privacy and shame; to see the human body as a thing to be examined, tested and studied.

One danger of this transformation is that you will see your colleagues and superiors do horrible things and be afraid to speak up. But the more subtle danger is that you will no longer see what they are doing as horrible. You will just think: This is the way it is done.

Jessica Lange Says Delayed “Long Day's Journey Into Night” Film Is 'Finally Finished' (Exclusive)

The long wait for the Long Day's Journey Into Night film might be finally over, Jessica Lange tells PEOPLE.

Filming for the new movie adaptation of Eugene O'Neill's Pulitzer-winning play wrapped in November 2022, Deadline reported, with Lange reprising her role as morphine-addicted matriarch Mary Tyrone — the part she first played in the 2016 Broadway revival to great acclaim (and to her first Tony Award ).

Like that production, this Long Day's Journey Into Night was primed to be a major awards contender. Not only was it stacked with a superstar cast (like Ed Harris as Mary's husband James, and Ben Foster as their alcoholic elder son, Jamie), but it came from theater director Jonathan Kent, in his film debut.

But a release date for the film has yet to be announced.

Now, two years later, that end looks near. "It's finally finished," Lange, 75, tells PEOPLE at the 2024 Tony Awards Meet the Nominees press junket, where she was celebrating her leading actress Tony nomination for her role in Mother Play . "You'll be able to see it in theaters."

She went on to say that financial problems factored into the delay.

"It was a project that we all loved and were passionate about, but the money was never fully there so it was always scraping together a little bit here, a little bit there," Lange says. "And I think now, maybe, we finally have finished it. Hopefully!"

Financial problems have plagued the Long Day's Journey Into Night movie since filming began in Wicklow, Ireland in September 2022.

A day after cameras went up, production shut down, The New York Times reported, when lead producer Gabrielle Tana discovered their biggest chunk of financing had fallen through. The cast then spent weeks waiting patiently as Tana got the money to start again,

“We were shocked at first, of course, but never once did we think it wasn’t going to happen," Lange told the outlet.

The down had it's benefits, especially when it came to the performance. "As tough as it was when the money fell out, it was the most rewarding film acting experience I’ve had in quite a while,” said Harris, explaining that he was able “to sit back, think about the character, calm down, and just be this dude rather than worrying about playing such a classic, important role.”

They also were able to bond as a cast. "We just hung in, went to the pub, took long walks," explained Lange. "We really became friends and cared deeply for one another, because we were going through the same thing. I think that in some way it added to our intensity and passion for doing this."

First adapted for the screen in 1962 with Katharine Hepburn as Mary, the play has been made into four films, the last a 1996 film from Canadian director David Wellington.

Lange played Mary initially in a 2000 revival in London that later moved to Broadway.

In the 30 plus years since Lange made her Broadway debut, she's finally doing something for the first time on stage: originating a role.

"It's never been done before," Lange tells PEOPLE. "So of course, going into a piece like this, you wonder if it's going to work. But it's been wonderful. And then to be recognized for the work? I'm thrilled."

The 2024 Tony Awards will take place on Sunday, June 16 at Lincoln Center's David H. Koch Theater in New York City. Viewers can watch the show on CBS and Paramount+ beginning at 8 p.m. ET. Select awards will also be handed out on a preshow that will stream on Pluto TV.

WASHINGTON (AP) — President Joe Biden and former President Donald Trump on Wednesday agreed to hold two campaign debates — the first on June 27 hosted by CNN and the second on Sept. 10 hosted by ABC — setting the stage for their first presidential face-off to play out in just over a month.

The quick agreement on the timetable followed the Democrat’s announcement that he would not participate in fall presidential debates sponsored by the nonpartisan commission that has organized them for more than three decades. Biden’s campaign instead proposed that media outlets directly organize the debates between the presumptive Democratic and Republican nominees .

The debate is so unusually early on the political calendar that neither Biden nor Trump will have formally accepted his party’s nomination.

Hours later, Biden said he had accepted an invitation from CNN, adding, “Over to you, Donald.” Trump, who had insisted he would debate Biden anytime and anyplace, said on Truth Social he’d be there, too, adding, “Let’s get ready to Rumble!!!” Soon after that, they agreed to the second debate on ABC.

“Trump says he’ll arrange his own transportation,” Biden wrote on X, working in a jab about the perks of incumbency. “I’ll bring my plane, too. I plan on keeping it for another four years.”

The swiftness with which the matchups came together reflects how each of the two unpopular candidates thinks he can get the better of his opponent in a head-to-head showdown. Trump and his team are convinced the debates will exacerbate voters’ concerns about Biden’s age and competence, while Biden’s team believes Trump’s often-incendiary rhetoric will remind voters of why they voted him out of the White House four years ago.

The presidential debates, always a critical moment on the political calendar, could be particularly important in a year when voters are underwhelmed with their choices and have expressed concerns about the candidates’ advanced ages — Biden is 81 and Trump 77.

Sprightly on social media, the rivals traded barbs — each claiming victory the last time they faced off in 2020.

“Donald Trump lost two debates to me in 2020, since then he hasn’t shown up for a debate,” Biden said in a post on X. “Now he’s acting like he wants to debate me again. Well, make my day, pal.”

Trump, for his part, said Biden was the “WORST debater I have ever faced - He can’t put two sentences together!”

The June debate is likely to cap a busy and unsettled stretch, following the likely conclusion of Trump’s criminal hush money trial in New York , foreign trips by Biden to France and Italy, the end of the Supreme Court’s term, and the expected start of two criminal trials for the president’s son, Hunter Biden.

CNN said that its debate would be held at 9 p.m. ET in its Atlanta studios with no audience present in a break from recent precedent. Moderators will be anchors Jake Tapper and Dana Bash, CNN said. ABC did not offer details on where its event would be held, but the network said it would be moderated by anchors David Muir and Linsey Davis. Disagreements about moderators and rules were some of the questions that prompted the formation of the Commission on Presidential Debates in 1987.

The two campaigns and television networks had held weeks of informal talks on ways to circumvent the commission’s grip on presidential debates following years of complaints and perceived slights, according to two people familiar with the matter who spoke about internal discussions on condition of anonymity.

Biden’s campaign had proposed excluding third-party candidates, such as Robert F. Kennedy Jr. , from the debates outright. Under the debate commission’s rules, Kennedy or other third-party candidates could qualify if they secured ballot access sufficient to claim 270 Electoral Votes and polled at 15% or higher in a selection of national surveys.

Both CNN and ABC announced the same qualification threshold, saying candidates will need to reach at least 15% in four separate national polls of registered or likely voters that meet their standards.

In response, Kennedy accused Biden and Trump of “trying to exclude me from their debate because they are afraid I would win.” He said, “Keeping viable candidates off the debate stage undermines democracy.”

The debates will be the first televised general election matchups to be hosted by individual networks. The 1960 debates, which helped show the power of the medium to influence public opinion, were hosted jointly by the leading networks of the day. Before the commission was formed in 1987, the presidential debates of 1976, 1980 and 1984 were organized by the League of Women Voters.

Plans for a vice presidential debate have yet to be announced.

Trump has been pushing for more and earlier debates, arguing voters should be able to see the two men face off well before early voting begins in September. He has even proposed a debate outside the Manhattan courthouse where he is currently on trial. He also has been taunting Biden with an empty lectern at some of his rallies.

In a memorandum to Biden campaign chair Jen O’Malley Dillon on Wednesday, Trump senior campaign advisers Chris LaCivita and Susie Wiles challenged Biden to agree to at least two additional debates, suggesting one be held each month, with events in June, July, August and September, in addition to a vice presidential debate.

“Additional dates will allow voters to have maximum exposure to the records and future visions of each candidate,” they wrote.

Trump later posted on Truth Social that he had agreed to a third debate, this one hosted by Fox.

“Please let this TRUTH serve to represent that I hereby accept debating Crooked Joe Biden on FoxNews. The date will be Wednesday, October 2nd. The Hosts will be Bret Baier and Martha MacCallum. Thank you, DJT!” he wrote.

O’Malley Dillon responded with a statement accusing Trump of having “a long history of playing games with debates: complaining about the rules, breaking those rules, pulling out at the last minute, or not showing up at all.”

“No more games. No more chaos, no more debate about debates. We’ll see Donald Trump on June 27th in Atlanta – if he shows up,” she wrote.

In an interview with conservative radio host Hugh Hewitt Wednesday morning, Trump had raised his own doubts about whether Biden would show, and offered his own suggestions. He said the debates “should go two hours” with both men standing, and he also pushed for larger venues.

“It’s just more exciting,” he said.

Biden’s campaign has long held a grudge against the nonpartisan commission, accusing it of failing to evenly apply its rules during the 2020 Biden-Trump matchups — most notably when it didn’t enforce its COVID-19 testing rules on Trump and his entourage.

O’Malley Dillon on Wednesday sent a letter to the Commission on Presidential Debates to say that Biden’s campaign objected to its proposed debate dates in the fall, which would come after some Americans begin to vote, repeating a complaint also voiced by the Trump campaign. She also voiced frustrations over past rule violations and the commission’s insistence on holding the debates before a live audience.

“The debates should be conducted for the benefit of the American voters, watching on television and at home — not as entertainment for an in-person audience with raucous or disruptive partisans and donors,” she wrote.

There also was little love lost for the commission from Trump, who objected to technical issues at his first debate with Democrat Hillary Clinton in 2016 and was upset when a 2020 debate with Biden was canceled after the Republican came down with COVID-19. The Republican National Committee had already promised not to work with the commission on the 2024 contests.

The Trump campaign issued a statement on May 1 that said of the debate schedule offered by the commission: “This is unacceptable.”

The commission said in a Wednesday statement, “The American public deserves substantive debates from the leading candidates for president and vice president.” It said its mission is “to ensure that such debates reliably take place and reach the widest television, radio and streaming audience.”
