A GP visit card is a card that gives you free visits to a participating family doctor (GP). If you are not eligible for a medical card , you may be eligible for a GP visit card.

What does a GP visit card cover?

Your GP visit card covers the cost of visits to your GP and visits to GP out-of-hours services . Blood tests to diagnose or monitor a condition are covered.

The GP visit card does not cover hospital charges . Prescribed drugs are not free but may be covered by the Drugs Payment Scheme .

You can apply for a GP visit card online.

You must be ordinarily resident in Ireland to apply for a GP visit card. This means that you are living in Ireland and intend to live here for at least one year.

You will qualify for a GP visit card with no means test if you are :

  • Aged under 8 (see GP visit cards for children )

Aged over 70

  • Getting Carer’s Benefit or Carer’s Allowance , at full or half-rate

If you are aged under 70, your income is assessed by the HSE to see whether you qualify for a GP visit card. See below for information about the HSE basic rates of income.

Your income is assessed by the HSE using the means test for people aged under 70 . However, the basic rates of income for the GP visit card are higher than the limits for the medical card.

Your weekly net income is compared to the HSE's weekly basic rate of income . This is your income after tax, PRSI and universal social charge (USC) have been deducted. There are extra allowances for children.

If your income is above the weekly basic rate you may still qualify for a GP visit card after you include allowable expenses such as childcare and rent (see ‘Allowable expenses’ below).

The weekly basic rates of income increased on 11 September 2023 and again on 13 November 2023. If you were not eligible in the past, you may be eligible now.

Allowable expenses

Some living expenses are allowable. This means they increase your weekly basic rate of income.

Allowable expenses include:

  • Childcare costs
  • Rent (not including any amounts paid by Housing Assistance Payment or Rent Supplement )
  • Reasonable mortgage payments on your family home and other land or property.
  • Mortgage protection insurance and associated life assurance.
  • Home insurance.
  • Maintenance payments you make.
  • Nursing home, private nursing or home care costs for you or your spouse.
  • The cost of public transport.
  • Driving expenses if a car is required, at a rate of 30 cent per mile/18 cent per km. If a couple needs two cars to travel to work, a double allowance applies. The cost of parking can be taken into account.
  • Reasonable contributions towards carpooling costs.

Aged over 70 with dependants

If you are aged over 70, you are eligible for a GP visit card regardless of your income.

If you are over 70 and you have dependants who are aged under 70, for example, a spouse or partner your dependants may qualify for a GP visit card if your combined gross income is over €1050, but not greater than €1,400 a week.

If either of you are aged over 70, you will both qualify for a full medical card if your combined gross income is €1050 or less per week.

Finances over the qualifying financial threshold

When you apply for a GP visit card, the HSE will first assess your application for a medical card . If you don't qualify for a medical card, you will then be assessed for a GP visit card.

If your finances are over the qualifying financial threshold for a medical card or GP visit card, you may still qualify for a discretionary medical card if your medical expenses would cause you financial hardship without one.

If you want to apply for a discretionary medical card , you should also include information about your family’s medical expenses in your application.

Aged under 70

If you are under 70, you use the same application process to apply for a GP visit card as for a medical card . So, as part of the application process for the GP visit card, your entitlement to a medical card is automatically assessed.

Application for a spouse or partner

If you are over 70, with a dependant spouse or partner who is under 70, and you think your finances may be under the qualifying financial threshold, you can apply for GP visit cards for both of you. To do this, you use the standard medical card/GP visit card application form (pdf) and include details of your income and allowable expenses. (For the HSE weekly basic income rates, see ‘Dependants of people aged over 70’ above).

Applying if you are a carer

If you get Carer’s Benefit or Carer’s Allowance, at full or half-rate, you are eligible for a GP visit card. You can register for the GP visit card for carers by email or post .

Appealing your GP visit card application

If your application for a GP visit card is refused, you will receive a letter from the HSE to let you know. The letter will also set out the reasons why your application has been refused.

If you are not satisfied with the decision, you may have it reviewed. Your circumstances may have changed, or you may have left out some relevant information from the original application.

If you are not satisfied with the review, you can make an appeal to the Appeals Office of your HSE Area . You can also find the contact details for the Appeals Office in the letter of refusal from the HSE.

The Appeals Office will conduct a reassessment of your application. This will be conducted by HSE staff who were not involved in deciding on your original application.

Your entitlement to a GP visit card is reviewed periodically by the HSE. This is because your circumstances may change. If you don’t return your review form, your GP visit card may not be re-issued.

If you return your review form by the given date but the review process continues past the expiry date of your card, the HSE may extend your card’s validity so that you can continue to use it while the review is taking place. The extension is on a month-by-month basis, so it is advisable to contact the HSE Client Registration Unit (contact details below) to confirm that your card has been extended and continues to be valid.

If you're reading this there's almost a one in two chance you have either a medical card or a GP card. Yes, out of a population of 4.8 million, 2.1 million people, or 44 per cent of the population, have in their wallet one of those benefits even though, as is shown below, income limits for entitlement are very tight.

The number of medical card holders has grown rapidly in recent years, up from about 1.2 million in 2012 to 1.6 million as of July 2017.

And the figure would be even higher if the government had not pulled back on these benefits in 2009, introducing a means test for the over-70s, which was subsequently tightened in 2014.

There have been delays of late in granting medical cards due to staff shortages at the HSE. In the meantime TDs continue to lobby on behalf of their constituents; in June and July just gone almost 40 queries were made in the Dáil to Minister for Health Simon Harris on the status of individuals' medical cards.

THE BENEFITS

Free healthcare

A medical card entitles the holder to free GP services and out-patient services in public hospitals. Someone with a GP card is restricted to doctor visits only, and receives no recompense on hospital charges. Ordinarily a GP visit would cost someone between €50-€60. A visit to A&E, which typically costs €100, is also free for medical card holders.

Medical card holders can also avail of all in-patient services in public wards in public hospitals, including public consultant services, without having to incur charges of €80 a night, up to a maximum of €800 in any year.

Low-cost prescriptions

If you have a medical card you also won’t need to avail of a drugs payment card to reduce your monthly prescription bill as medical card patients pay only €2.50 per prescription.

Cheaper teeth, eyes and ears

A visit to the dentist is free for medical card holders and those who pay PRSI alike. However, medical card holders are also entitled to two fillings in each calendar year, as well as any extractions that are required.

One first-stage endodontic (root canal) treatment is also available each year for teeth at the front of the mouth, as are denture repairs, although other treatment will require HSE approval.

Medical card holders are also entitled to a free examination of their eyes, and any necessary standard spectacles once every two years. You can also get your ears tested for free, and will be entitled to a free hearing aid every two years.

When it comes to community care services, medical card holders are entitled to these free of charge, as are those without medical cards, but the distinction is that medical card holders get priority to these services as they have a legal entitlement.

Pay less tax

Holders of medical cards benefit from a lower rate of universal social charge (USC) on their income. Typically, once your income passes €13,000 you will incur USC at a rate of up to 11 per cent on your income. Holders of a medical card, however, will only pay a rate of 2.5 per cent on all income over €12,012.

However there is a caveat; to benefit from this lower rate of USC your income must be €60,000 or less if you are aged under 70.

Of course given the means tests that is in place to qualify for a medical card it’s likely that the cost of this tax saving is not significant.

Nonetheless it does mean that someone earning €60,000 who has a medical card will only pay USC of €1,500 on this income compared with €2,290 for someone unable to avail of the reduction. That’s a saving of €790 a year.

People who hold a GP visit card, a Drugs Payment Scheme Card, a European Health Insurance Card or a Long-term Illness Scheme Card do not qualify for the reduced rate.

Free school transport

If your children attend a primary school located more than 3.2km from your home you may be entitled to discounted transport to attend the school. Typically,a charge of €100 per year applies for the service, capped at €220 per family, but if your child has a medical card you will not have to pay the charge.

Free exam fees

Students who are taking or repeating the Leaving Certificate must pay examination fees in order to sit the exams. The fees range from €116 for the first time sitting the exam to €301 for repeating it. If you or your parent(s) or guardian(s) hold a medical card you are exempt from these fees.

Medical card holders (or children of them) are also exempt from the €200 contribution for attending a post-Leaving Certificate college.

Cheaper childcare

Under the Community Childcare Subvention (CCS) programme, which is aimed at supporting disadvantaged parents, medical card holders may be entitled to cheaper childcare for their children provided that the childcare provider is registered with Tusla.

The rate of reduction will depend on the parents’ circumstances. For example, parents or guardians who have a medical card but also get most social welfare payments and/or are enrolled in certain employment schemes, can get a weekly reduction of €145 for a full-time place (€580 a month).

Parents with a medical card but without social welfare payments are still entitled to a monthly reduction of €280 for a full-time place.

Parents with a GP visit card, where the child is older than six, are entitled to a monthly reduction of €200 for a full-time place.

Holders of a medical card are also entitled to a maternity cash grant of €10.16 on the birth of a child.

HOW MUCH DO THEY COST THE STATE?

Unsurprisingly, given the wide range of benefits they offer, medical cards are expensive. Figures from the HSE show that the cost of a medical card for someone aged 70 or over typically comes to about €2,092 a year – and 20 per cent of those who hold a medical card are in this age group.

Children aged four or younger will have a cost of €392 for a medical card, or €243 for a GP card, while those in the 44-65 age group will have an indicative annual cost of €1,217.

These costs, however, only relate to payments for GP, pharmacy, dental and optical services, so the true costs – when the lower rate of USC plus added services such as free school bus are factored in – are likely to be considerably higher.

Medical cards are granted on the proviso that they should be handed back should an individual’s circumstances change, although there may be a grace period for up to three years. It’s up to the holder to notify the HSE. And the HSE doesn’t keep track of how many cards are handed back.

According to a spokeswoman, the Primary Care Reimbursement Service (PCRS) does not routinely collect data on the number of people who return their medical cards, although she did note that last year the National Medical Card Unit undertook 482,125 full reviews and self-assessments.

HOW DO YOU GET ONE?

The aim of a medical card is to predominantly assist low-income families meet the cost of healthcare. This means that income limits are stringent.

For example, a single person living alone should not have income above €184 a week, or €164 if they have a family, although if their total earnings exceed this, and are drawn only from welfare payments, they will still qualify.

A couple can earn €266.50, with allowances available per child, so a couple with three children can earn up to €383.50 a week (€19, 942) and still qualify, although in reality the income limits may be higher as “reasonable expenses”, such as rent, childcare and travel expenses, will also be allowed.

In addition, savings and investments are also taken into consideration, although income of €36,000 a year (individual) and €72,000 (couple) are disregarded, while the value of a family home is not assessed.

Higher income limits apply to the GP card.

For the over-70s different rules apply. Of course not so long ago everyone in this age category received a full medical card, but since 2009 a means test applies which means that many will now only qualify for a GP card.

Typically, to qualify you should not have a pre-tax weekly income (pensions, savings, investments and some property are take into account) of more than €500 for a single person or €900 a week for couples, down from €600 and €1,200 a week respectively from January 2014.

However you might still qualify on hardship grounds for a so-called “discretionary card”. According to Age Action, there are now about 100,000 such cards in use, which means that income is not a barrier to getting a card depending on the circumstances of your health.

A report from the ESRI in 2015, for example, found that one in five of those in the highest income decile (the top 10 per cent of the population by income) reported having a medical card even though very few of this group are modelled as having an entitlement to a medical card

From today, around 78,000 additional people will have access to free GP visits .

Children aged six and seven years old are now eligible for free GP care.

The scheme is part of an expansion in access to free GP care that will eventually cover around half a million people in Ireland.

In the next few months, free GP care will be expanded to people who earn up to the median income.

How to access free GP care for your child

If you child currently has a GP visit card, it will be automatically extended as valid until the child's eighth birthday.

Parents can register their children for a GP visit card by visiting the HSE website on hse.ie/gpvisitcards.

Applications take between seven and 10 working days to process.

It is also possible to register by post.

In order to register you will need the follow three pieces of information:

  • Your PPS number
  • Your child's PPS number
  • Your GP's name

You can check if your GP is accepting patients on this list .

What does the GP visit card cover

The GP visit card for under-eights covers:

  • Free GP visits
  • Assessments at ages two and five
  • Out-of-hours urgent GP care
  • Care for children with asthma

A 'significant milestone'

Minister for Health, Stephen Donnelly said the expansion of free GP care to under-eights is a "significant milestone".

"I am committed to ensuring that cost will not be a barrier for children’s healthcare," he said.

"Ensuring that children in Ireland can visit their GP for free gives them access to healthcare at the right time to help ensure their long-term health and wellbeing."

He also said that the number of trainee general practitioner doctors will have increased from 900 per year to 1,300 by 2026.

GP staffing issues have long been an issue in Ireland.

Two weeks ago it was announced that pharmacists may take on some GP roles and university places for medical degrees will be increased in an effort to relieve the pressure on GPs.

Medical cards vs gp visit cards: what to know if you’re returning to ireland.

When people return to Ireland from the UK, questions about healthcare benefits are usually on the top of their list. There is usually confusion between medical cards and GP cards. 

You need to be ‘ordinarily resident’ in order to have either of the cards, that means you need to show you live in Ireland and intend to do so for a year or more.

A lease or proof of owning a property or, in some cases, an affidavit will be enough to prove this. This means that, even if you have just arrived, you can still be found to be ordinarily resident immediately, so long as you can show you are planning to stay for at least a year and intend to spend at least 8 months of that year actually living here. 

Medical cards

A medical card entitles you to access certain health services free of charge. It is almost always means–tested, regardless of your age. However, once you are over 70 years old, the medical card means test changes and becomes more lenient.

If you are ordinarily resident in Ireland but are getting a social security pension from another EU/EEA country or the UK or Switzerland, or you are working and paying social insurance in one of these countries, you may be entitled to a non–means–tested medical card under EU rules. You must not be in receipt of a contributory Irish social welfare payment or be working in Ireland and liable to pay PRSI, in order to qualify. 

GP Visit Cards

A GP visit card does exactly what it says on the tin. It entitles you only to see your GP free of charge. It is means–tested if you are between 6 and 69 (inclusive). The means test for the GP visit card is different to the test for the medical card, so if your income is low, but slightly too high for a medical card, you may be offered a GP visit card instead. Children under 6 and adults who are 70 or older are entitled to a non–means–tested GP visit card, so long as they are ordinarily resident in Ireland.
